Hyperbaric Oxygen Therapy

CMS-COVERED INDICATIONS

  1. Acute carbon monoxide poisoning

  2. Decompression sickness

  3. Gas embolism (arterial air embolism)

  4. Gas gangrene (clostridial myonecrosis)

  5. Acute traumatic peripheral ischemia (including crush injuries and compartment syndrome)

  6. Compromised skin grafts and flaps

  7. Chronic refractory osteomyelitis (unresponsive to conventional management)

  8. Delayed radiation injury (soft tissue and bony necrosis, including osteoradionecrosis)

  9. Diabetic wounds of the lower extremities meeting all of the following:

    • Type I or Type II diabetes

    • Wagner Grade III or higher

    • Failed an adequate course (≥30 days) of standard wound therapy

  10. Severe anemia when transfusion is not possible or contraindicated

  11. Actinomycosis, refractory to antibiotics and surgical management

  12. Intracranial abscess

  13. Necrotizing soft tissue infections

Required Documentation for Referral
  • Patient demographics​ 

  • Insurance information

  • Most recent progress note

  • Relevant imaging / vascular studies (if applicable)

  • Wound measurements & duration (for DFU cases)

  • Culture results (if osteomyelitis suspected)

What Happens Next (we handle all these)

  1. Insurance verification

  2. Prior authorization initiated (if required)

  3. Evaluation scheduled (goal: 2-4 business days)

  4. Consult note sent to referring provider
